4.1.2.06. Rating - Opinion survey

The following is provided as and aid to evaluate this step of the improvement process. All or most items need to be considered to allow final judgment but not all may apply to the specific client situation.

Main purpose of step

To collect subjective/objective information about the actual situation/culture as relevant to the improvement process. To also "loosen" up the organization for the improvement process to come.

Aspects to consider:

bulletopinion survey done (not less than 3 years ago)
bullet

formal questionnaire used to include:

bulletfactual information on performance
bulletsubjective information on quality aspects
bulletuse of numbers to allow quantification of opinions
bulletall levels in organization involved
bulletsample used representative for population
bulletcarried out by unbiased person
bulletanonymity
bulletdone written and/or verbally
bulletresults analyzed and published
bulletaction plan prepared (as applicable) and executed

 Score 0 - 5

Examples of opinion survey questionnaires are available for three organization levels: (top and middle) management, (direct) supervision and (operational) employees
