Safety Opinion Surveys

Included here are three PDF documents that can be used to carry out opinion surveys among three groups in the organization: safety opinion survey management, safety opinion survey direct supervision and safety opinion survey operational employees. I used these surveys prior to doing safety audits with the International Safety Rating System but they can be used on their own - just to get an impression of how people at different organizational levels feel about the safety efforts of their organization. In particular this can be an eye-opener for management and I have had these experiences where there were quite some differences between how management felt about their safety program and the way the operational employees looked at it.
