Loss Control Maturity Profile

This Loss Control Maturity Profile was based on the profile that Phil Crosby presented in one of his books on quality. I used this as the basis as a "safety culture" indicator to trigger discussions between people attending training sessions. Normally during training you will only get the opinions of the dominant few. By giving each of the people a sheet to give their own opinion, you can afterwards collect the sheets again and - after some work - present them with the total results of the group. You can computerize this to allow immediate feedback that will stimulate discussion that is often required to initiate an improvement process. You can put numbers on it for quantification and carry the exercise out at different levels in the organization to see any differences that may be there. This is a PDF document.
